BACKGROUND

The effects of intensive glucose control on cardiovascular events in patients with long-standing type 2 diabetes mellitus remain uncertain. (N Engl J Med 2009;360:129-30.)

remain uncertain 　不明である
　be動詞ではなくremainを使って、「ずーっとそうだった」という印象を強めています。ちなみに、ロングマン現代英英辞典４訂増補版では次のように解説しています。

remain to continue to be in the same state or condition

METHODS

We randomly assigned 1791 military veterans (mean age, 60.4 years) who had a suboptimal response to therapy for type 2 diabetes to receive either intensive or standard glucose control. (N Engl J Med 2009;360:129-30.)

suboptimal 次善の、準最適の
　コントロールが今一つ、ということです。

The primary outcome was the time from randomization to the first occurrence of a major cardiovascular event, a composite of myocardial infarction, stroke, death from cardiovascular causes, congestive heart failure, surgery for vascular disease, inoperable coronary disease, and amputation for ischemic gangrene. 　なんでもない","ですが、that is (すなわち）という意味をもって、ここに打たれています。","以下にa major cardiovascular eventの内容が記載されています。

RESULTS

There was no significant difference between the two groups in any component of the primary outcome or in the rate of death from any cause (hazard ratio, 1.07; 95% CI, 0.81 to 1.42; P=0.62). (N Engl J Med 2009;360:129-30.)

poorly controlled　コントロール不良の
　METHODSのところで出てきたsuboptimalと同義です。言い換えて使っていますね。

長期にわたる血糖コントロール不良ですでに心血管系にダメージが蓄積されている場合にはそこから厳格にコントロールしてももはや遅し、ということでしょうか。「早期からのコントロール」が重要なんでしょうね。
